Job Description
SCOPE OF POSITION
The RN - Disease-Specific Program Manager, in collaboration with the administrative leadership, will implement and maintain the Sepsis Program. The philosophy, goals, and objectives are congruent with the mission and objectives of Ballad Health. The Manager is responsible for development, implementation, marketing, and over all clinical flow of assigned Disease-Specific Program/s. They have the responsibility to develop a communications network with the emergency department, appropriate nursing units, physicians, and referring hospitals as applicable. The Manager also assists team members in coordinating cases through working closely with team members, physicians, system, and outlying referral hospitals. They also work actively on development of process improvement initiatives as it relates to each program. This individual serves as the interdisciplinary team leader to provide management of the patient's care from pre-admission to post discharge. The manager will manage the quality of clinical management, effective discharge planning process and patient/family education process.
The RN-Disease-Specific Program Manager is accountable for practicing fiscal responsibility by working with other department managers in developing realistic annual operating and capital budgets, conserving resources, and monitoring expenditures to ensure they remain within budgeted parameters.
The Manager is responsible for implementing program changes to ensure program success as patient or physician's needs change and/or any regulations associated with the program change. They will be involved with collecting and compiling outcomes data and assisting with implementing changes to processes for quality improvement and patient satisfaction.
The Manager must possess excellent interpersonal communication skills and function as a liaison between physicians, interdisciplinary treatment team, and the community in general, as well as all aspects involved with marketing the programs. They will be responsible for keeping all marketing and education materials up to date and in good supply. The Manager is expected to set an example for all employees of commitment and loyalty to Ballad Health by openly supporting and participating in hospital projects and events. The person in this position must be able to work as a team player due to the very nature of the job by communicating effectively with team members and providing open dialogue for solutions. The ability to speak before groups is essential.
This person should be committed to continuing education and current information to keep abreast with the very changing demands of the position and developments in health care. It is vital that an individual in this position be capable of good communication skills. It is of the utmost importance that written communication is legible.
Job duties of this position may require access to protected patient information (PHI). The team member will be accountable for appropriate use of the record and compliance with all confidentiality and security policy and procedures related to use, access, and disclosure of PHI.

EDUCATION AND EXPERIENCE
Graduate of an accredited school of professional Nursing with current Registered Nurse license in the appropriate state.
Three years experience in hospital setting.
Excellent leadership skills.
Ability to function independently with minimal supervision.
Excellent communication, problem solving skills, decision-making skills, critical thinking skills.
Capable of accumulating and analyzing data, recognizing opportunities for improvement, and formulating and implementing plans for change.
Ability to develop positive relationships with physicians, community, and co-workers.

Job Type:
